We found out on Friday that CEVA requires all players to turn in both page 1
and 2 of the CEVA registration, we have page one already.  We need your help
collecting page 2 "The Waver & Release of Liability" for each player.

 

PLEASE COMPLETE ASAP:

 

1.	Log in to your account
2.	Click "My Information
3.	Click "Membership Info"
4.	In the "Membership History" section click the icon   "

Confirmation"Looks like paper, for your current membership paper work.
5.	Print/save page "2 - WAVER AND RELEASE OF LIABILITY" as a PDF and
email to  <mailto:info@pdx-vb.com> info@pdx-vb.com 

a.	Second option; fax to 503-296-2017
b.	Third option; deliver directly to your coach at the next practice
c.	Forth option; reply to this email with your log on and password to
CEVA and we will print the waver for you.

 

Players are technically not allowed to practice and will not be allowed to
play in tournaments until this is completed!  We appreciate your
understanding and help completing the paper work.
